LEARNING EVENT 4:
SUBMIT A MAINTENANCE REQUEST
As the NBC defense officer or NCO, you may be required to submit, or check the submission of,
a maintenance report (DA Form 2407). At the organizational level, DA Form 2407 is used to
request repairs and maintenance services from a higher-level maintenance facility. It also is used
to report accomplishment of a Modification Work Order(MWO), to submit an Equipment
Improvement Recommendation (EIR), and to report receipt of defective material by your unit.
The form may be used to record accomplishment of maintenance actions. For your purposes,
you will be mainly concerned with requesting repairs and maintenance services. The instructions
below tell how to fill out a DA Form 2407 (figure 10 on page 40). You will be concerned
mainly with blanks 1 through 16. Not all these blanks will apply to the equipment which you are
submitting for maintenance.
Preparation
The blocks on the DA Form 2407 are filled in, as applicable, according to the following
instructions.
Heading. Put a check or an "x" in the block to identify a work request Put a priority code
(obtained from unit supply) in the block labeled "ORG PD" Obtain the commander's signature if
the priority code is 01 through 08.
Page Number and Total Pages. Leave these spaces blank. These spaces are provided for
entering the page number and total pages when entries in section II are complete.
Control Number. A control number has been preprinted in the control number block.
Block 1a. Enter the name of the organization submitting the report.
Block lb. Enter the location of the organization initiating the request; units overseas enter APO
only.
Block 1c. Enter the unit identification code of the unit identified in block 1 a. If you do not
know this information, you can obtain it from the orderly room or unit supply.
Block 2. Enter the serial number of the item. When preparing this form for multiple items (for
example, protective masks) leave this block blank.
Block 3. Enter the noun abbreviation of equipment for which the form was initiated.
