LEARNING EVENT 3:
INITIATE A SUPPLY REQUISITION REQUEST
When you discover a difference between the amount of NBC supplies and equipment authorized
and the amount which is on hand, you must request initiation of a supply action to balance the
numbers. Usually, this will be accomplished by submitting a disposition form to the unit supply
or Battalion S4 who will prepare a DA Form 2765-1, Supply Requisition. There are several
reasons for initiating an action in this way. It is important that you understand that the unit NBC
defense program is NOT your sole responsibility. An effective unit NBC defense program is a
collective effort of the entire unit. By your coordination with appropriate elements responsible
for various actions, you will, in fact, promote the success of the program by making them
become involved. You will also find that you have more success in receiving requested items
when the actual request is made out and processed by those who are trained to do it.
Not all requests for supply action will be a request for supplies or equipment. If, as a result of
your physical inventory, you find that you have more of an item than authorized or required, you
should initiate a request to have the excess items turned in. Another type of supply action is a
request for logistical services.
Request NBC Supplies or Equipment
Proper stockage of NBC supplies is the result of accurate accountability and timely submission
of supply requests. A request for supplies should be submitted as soon as a shortage is detected,
or when it is known that a shortage will exist at a known future date (for example, when you
know that on-hand items will be used for training which is scheduled, or as in the case of
Chemical Agent Detector Kits, which you know will expire soon). To insure that serviceable
M256 Chemical Agent Detector Kits are always on hand, a new kit should be requisitioned six
months before the current kit is due to expire.
When the need for a requisition is identified, it is normally initiated by submitting a DA Form
2496, Disposition Form to the unit supply or the Battalion S4 requesting the needed supplies be
placed on requisition (figure 9 on page 38). The request must identify the name of the item,
National Stock Number (NSN), and the quantity required. It is important that you retain a copy
of the request for your files. When the items are placed on requisition, you should write in the
requisition number for the item on your copy of the supply request If the item is not received in a
reasonable time, periodic checks must be made to insure the requisition is still valid.
