When used to request maintenance on more than one item, this space will be used to record
pertinent information regarding maintenance required, quantity of items requiring maintenance,
and the serial number of the items, if available. When requesting maintenance on several similar
type items with different stock numbers, you may use one DA Form 2407. In such cases, leave
block 6 blank.
